Urinary tract among females
UTI are common among females most of the time. Females who are sexually active and after menopause vulnerable to UTIs. As we all know It is a very common disease in community. The commonest presentation is back pain and dysuria for 2-3 days. In such presentation we have to first think about this if they are at that age groups.
When we talk about UTI we have to think about aetiology of disease. The sign sand symptoms of UTI may vary. Among them
- Pain or burning during urination
- The need to urinate more often than usual
- A feeling of urgency during urination
- Blood or pus in the urine
- Cramps or pain in the lower abdomen
- Chills or fever (fever may be the only symptom in infants and children)
- Strong smelling urine
- Pain during sexual intercourse
- Nausea, vomiting, and malaise can be seen.
In a case like that we have to take a short history from woman. within history we have to ask about aetiology, symptoms, and signs, past medical and surgical history
After taking history we have to do a abdominal examination. In abdominal examination most of the times we can find tenderness over the lower abdomen
If bladder is palpable in examination which indicate urinary retention which take us to a different entity.
So our next target is prescribe them drugs
my prescription is
PCM 2tab* tds* 3days (can use Dic Na, ibuprofren etc.......)
Vit C 2tab* tds* 3days- urine acidification
Domperidone 10mg* tds* 3days- for nauseae and vommiting.
Ciprofloxacin/ norfloxaxin/ Ofloxacin/ Levofloxacin/ Cifixime/ Augmentine/ cefuroxime
Aug, cifixime, cefuroxime can be use in Diabetic patients
With empirical Tx we have to ordered UFR and Urine culture
Generally we have to advice drink more water
I have lot to say but we can talk about this subject more on next day
